• Guest Pass
  If a student from QHS would like to bring a guest to a school function, including dances, Prom, etc., a guest pass must be filled out and turned in to the office.  The guest must be under 21 years of age and will be expected to comply with the school policies governing QHS students, which are listed on the guest pass.

• Athletic Eligibility (Physical) Form
  An athletic eligibility form must be filled out and turned into the office before an athlete participates in a sport.  A form only needs to be filled out once a year.  For example, if a student fills a form out in the fall, the same form will still be valid in the winter and spring of that year.  *Note the front sheet of the form must be filled out twice.  

• Various Scholarship Application
  Applications are for local scholarships.  Please keep track of the application deadline!  If there are any questions about any of the applications or scholarships, please see Mr. Barnes as soon as possible!
